Wolford Clinic Monthly Massage Therapy Agreement

I agree to schedule monthly massage therapy sessions at the Wolford Clinic for one year in exchange for receiving a discounted rate.

Payment for monthly sessions will be as follows:60 Minute MassageFirst Month/Visit - $75 per hour
Second Month/Visit - $65 per hour
Subsequent Months/Visits - $55 per hour90 Minute MassageFirst Month/Visit - $90 per hour
Second Month/Visit - $80 per hour
Subsequent Months/Visits - $75 per hour120 Minute MassageFirst Month/Visit - $120 per hour
Second Month/Visit - $110 per hour
Subsequent Months/Visits - $100 per hour

All payments are due at the time of service.If two months pass without a scheduled massage therapy session, the rate will return to the standard rate for the requested session.Either party may terminate this agreement at any time for any reason.
